Medical Supervisor Registration

Registration is easy. Use the electronic form or fill out the PDF form and return it via email to Med-Supe@oehha.ca.gov.

We will confirm your registration by email and post your name on the list of registered medical supervisors.  This list is accessible to agricultural employers to identify registered Medical Supervisors in their area.
