Medical Supervisor Registration

Registration is easy.  Fill out the registration form below and return via email to Med-Supe@oehha.ca.gov.

We will confirm your registration and post your name on the list of registered medical supervisors.  This list will be available to the agricultural employers to help them locate local physicians who are qualified to be Medical Supervisors.
